This plan represents over six years of work and study by the Cape Care Coalition, a dedicated group of Cape Cod residents, health and human service professionals and public officials. This document is best seen as a blueprint for how a comprehensive and integrated system of health care could more effectively, efficiently and affordably improve the health of Cape Codders. Rather than our current market model of care delivery, in which each independent provider seeks to maximize utilization of whatever services they provide, we envision a model in which all benefit from a system that is purposefully organized and focused on maintaining health for all, and delivering coordinated, evidence-based and safety-focused care of illness, over our lifespan.

As this is a "living document", we encourage residents of the Cape to comment and suggest ways in which it can be improved. We welcome involvement, feedback, suggestions and questions.
